Q: Are you certified by any of the major accrediting bodies?
A: Yes. We are fully certified by NCQA and accredited by URAC.
Q: Where are you located?
A: Optum employs over 30,000 people in the U.S. and throughout the world. Our headquarters is located in Eden Prairie, Minnesota. Our main office for credentials verification is located at 9100 Shelbyville Road, Suite 250, Louisville, Kentucky, 40222.

Q: Why are you contacting me?
A: In order to participate with the health plan noted on your correspondence, we are providing notice to you that OptumInsight has started credentialing activities on its behalf.
Q: I am a member of CAQH. Why don’t you get my data from CAQH?
A: Please ensure that you have designated the health plan to use your CAQH information. Additionally, there may be additional information or documents that the health plan requires to complete their internal credentialing that may not be available on CAQH. Many items are also time-sensitive and may need to be updated since your last re-attestation on CAQH.
Q: What happens if I don’t complete this application?
A: Your participation and contract with the health plan may be in jeopardy if Aperture is not able to receive and complete the application processing on behalf of the health plan. Aperture does not make any determination on your approval or denial into the health plan’s network.
Q: I don’t affiliate with OptumHealth. Do I need to complete this?
A: Yes. OptumHealth is a completely separate business segment that has nothing to do with this credentialing request.
Q: Who can I contact about questions regarding this application?
A: Your health plan’s provider rep or call center.
Q: Is my data safe?
A. OptumInsight takes identity theft seriously. We only contract with health plans and security measures in place that have been approved by leading accrediting bodies. OptumInsight has never had a security breach on provider data records.
Q: Is there a cost to complete this?
A. No, there are no costs to any provider for this service.
